Old Irving Park Community Clinic (OIPCC) is a free medical clinic under Illinois law and any health care professional who, in good faith, provides medical treatment, diagnosis, or advice as part of the services of the clinic and receives no fee shall not be liable for civil damages as result of his or her acts or omissions in providing that medical treatment, except for willful or wanton misconduct.

To qualify for care at OIPCC you must meet these requirements:

1. You must have NO health insurance of any kind (Veterans Insurance, Medicare, Medicaid, Chips etc.).

2. Your income must be at or below 250 % of the federal poverty level.

Proof of Income: To qualify for services, you will need to show one of the following documents:

• Current W2/income tax forms

• Last two pay stubs/checks

• Signed and notarized letter from employer stating monthly, weekly or annual salary

• If you are supported by someone else, you MUST provide their proof of income.

3. You must be 18 years of age or older.

4. We do not provide any prenatal or obstetrical care.

5. You will also need to bring:

Picture identification: passport, ID, or driver’s license

Proof of address: utility bill, hospital letter or correspondence

If you qualify based on the above criteria – please call the Clinic at 773-427-0298 to complete a Phone Triage session and schedule an Intake Appointment. Patients are seen by appointment only.

We are open Monday through Friday, 9 to 5 by appointment. 

Please note, we require patients wear a mask while inside the Clinic.
